What is required to change an address on an account? – CX Procedure

  • Manual Demographic Change (OLB Instructions)
    • CX Must provide and request the following information:
      • Change of Address – (A physical address cannot be a P.O. Box; however, a mailing address can be a P.O. Box address)
        • Color copy of valid government-issued ID
          • If the new address is on the client’s driver’s license, we only require a color copy of a valid government-issued ID
          • If the new Address is NOT on the client’s driver’s license, the following documents will be requested – the document must include the client’s full name and new address
            • Bank Statement (checking, savings, or CD)
            • Utility Bill (water, gas, and electricity)
            • Voter Registration
            • Change of Address confirmation from the United States Postal Service (USPS)

        • Provide the client with the Account Maintenance form
          • Color copy of valid government-issued ID
            • If the new address is on the client’s driver’s license, we only require a color copy of a valid government-issued ID
            • If the new address is NOT on the client’s driver’s license, the following documents will be requested:
          • The document must include the client’s full name and new address
