Identity Theft Process

  • CX Team will
    • Provide Identity Theft Affidavit to the client or identity theft victim
    • Explain the affidavit requirements
      • Affidavit must be notarized
      • Include a color copy of valid government-issued ID
      • Proof of residence (copy of utility bill or bank statement)
      • Police report may be required, Quontic’s Fraud Department will request if necessary
      • Return by email to [email protected]
        • If the Affidavit is returned to the CX Team, forward it to Fraud@admin
      • Hubspot Template: Identity Theft Affidavit
  • CX Team will not
    • Disclose information about the account
    • Respond to account detail inquiries with a statement like the following: “I understand this is a stressful time, until we complete the investigation, we are not able to provide any details.”
  • Fraud Department will
    • Review the account, documentation, and other information as required to process the identity theft claim
