- Provide Identity Theft Affidavit to the client or identity theft victim
- Explain the affidavit requirements
- Affidavit must be notarized
- Include a color copy of valid government-issued ID
- Proof of residence (copy of utility bill or bank statement)
- Police report may be required, Quontic’s Fraud Department will request if necessary
- Return by email to [email protected]
- If the Affidavit is returned to the CX Team, forward it to Fraud@admin

- Disclose information about the account
- Respond to account detail inquiries with a statement like the following: “I understand this is a stressful time, until we complete the investigation, we are not able to provide any details.”
