What is required to change a Social Security Number on an account?

Manual Demographic Changes (Email Instructions)

• CX must provide and request the following information:
  • Change of Social Security Number
    • Provide the client with the Account Maintenance form
      • Client will include the correct Social Security Number following their name on Customer/Business Name line
  • • Color copy of valid government-issued ID
    • Color Copy of the Social Security Number
• Email Template: Demographic Change: Social Security Number – Documents Needed (Email Instructions)
  • Attach PDF of Account Maintenance Form
