Manual Demographic Changes (Email Instructions)

  • CX must provide and request the following information:
    • Change of Birthdate
      • Account Maintenance form
        • Client will include the correct birthday following their name on the Customer/Business Name line
        • Color copy of valid government-issued ID
  • Email Template: Demographic Change: Birthdate – Documents Needed (Email Instructions)

  • CX must provide and request the following information:
    • Change of Birthdate
      • Color copy of valid government-issued ID
